The Draught of Whispers Elixir, obtainable from Exposed Roots, Whispers Cache, and Coven’s Cache, boosts the chance of Fugitive Heads spawning by 50% and increases the likelihood of Forgotten Altars appearing.
Another critical factor is the game's difficulty level. Fugitive Heads have a very low spawn rate on difficulties below Torment, which is already challenging to obtain. Casual players might want to take their time adjusting to Torment difficulty before embarking on a Headhunt.
