Silent Hill f has become the first entry in the franchise to receive Japan's strictest 18+ rating certification.
The Japanese reveal trailer displayed ratings of Mature (US), PEGI 18 (Europe), and CERO:Z (Japan) — marking a franchise first. While ratings may change during development, previous Japanese-developed titles like Silent Hill 2 and Silent Hill 3 carried the milder CERO:C (15+), whereas Western-developed entries typically ranged between CERO:C and CERO:D (17+).
Notably, even Bloober Team's Silent Hill 2 Remake retained a CERO:C rating in Japan despite launching with Mature 17+ ratings elsewhere.Konami's recent Silent Hill Transmission dedicated its full runtime to Silent Hill f — the first mainline installment in 13 years. Announced in 2022, the game transports players to 1960s Japan, following Shimizu Hinako, a teenage girl buckling under societal pressures. The narrative comes from Ryukishi07, acclaimed writer of the When They Cry series.

The trailer depicts Hinako in visceral distress — bloodied, wounded, and clad in a tattered school uniform. Though gameplay remains unseen, the footage teases grotesque monster designs and a haunting Otherworld where blood-red flowers replace the series' signature rusted industrial aesthetic.
